Occupational Therapist

Department: Home Health
Location: Topeka, KS

We are seeking a skilled and compassionate Occupational Therapist to join our Home Health Team.

Summary:

The Occupational Therapist is responsible for the oversight of OT services ensuring the delivery of therapeutic interventions.

Employment Status: Monday-Friday 8am-5pm

Pay Rate: $40/hr.+ based on experience

Essential Duties & Responsibilities

  • Using all information sources available completes initial assessment, periodic assessments and annual assessments on participants’ functional ability and restorative potential.
  • Participates in the IDT communicates changes in the participant’s condition and assists with the Plan of Care.
  • Assists in 24-hour participant care delivery.
  • Documents in the participant record in a timely manner
  • Determines participant and family needs related to functional ability.
  • Confers with participant and family to identify participant goals and expectations.
  • Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant.

Qualifications

Required: Graduate of an accredited program in occupational therapy approved by the American Occupational Therapy Association. Kansas State license to practice as an occupational therapist in compliance with Kansas State board of healing arts. Current driver's license, reliable transportation, and valid automobile insurance required.

Preferred: One year of professional experience required, minimum one year experience working with the frail and elderly population, home care or geriatric experience.
